Services Eligibility
Housing Stabilization Services are available to individuals who meet all of the following criteria:
- Must be enrolled in Medical Assistance (MA)
- Be 18 years of age or older
- Have a documented disability or disabling condition affecting one or more of the following: communication, mobility, decision-making, or managing challenging behaviors
- Be experiencing housing instability
- Be at risk of institutionalization and meet the level of care required for one of the following waivers:
- Brain Injury (BI)
- Community Access for Disability Inclusion (CADI)
- Community Alternative Care (CAC)
- Developmental Disability (DD)
- Elderly Waiver (EW)
These criteria help ensure that individuals with the highest need receive the appropriate support to maintain stable housing.

Housing Stabilization Consultation
This service helps individuals without MA case management services create a person-centered plan tailored to their housing needs. The goal is to develop a customized strategy that identifies barriers, sets achievable housing goals, and outlines the necessary steps to secure and sustain stable housing. Housing Stabilization Consultation focuses on empowering individuals by connecting them with resources, supports, and services that align with their unique circumstances, ensuring a comprehensive approach to housing stability.
Housing Stabilization Transition
This service is designed to assist individuals in finding appropriate and secure housing. It includes support with navigating housing options, completing applications, understanding rental agreements, and overcoming barriers such as credit issues or past evictions. By providing hands-on guidance, this service ensures individuals can successfully transition from homelessness or unstable housing into a safe and permanent living environment.
Moving Expenses
Moving Expenses are an integral part of Housing Stabilization-Transition Services and are reimbursable to providers for specific costs incurred on behalf of individuals receiving these services. Eligibility applies to members transitioning from a Medicaid-funded institution or exiting a provider-operated living arrangement to establish their own independent residence.
All moving expenses must receive prior approval and be explicitly detailed in the Housing-Focused Person-Centered Plan, Coordinated Support Services Plan, or Coordinated Care Plan. For comprehensive details regarding member eligibility, qualifying living arrangements, covered and non-covered expenses, limitations, and provider responsibilities.
Housing Stabilization Sustaining
This service supports individuals in maintaining their housing long-term. It includes assistance with landlord communication, budgeting for rent and utilities, and accessing community resources to address ongoing challenges. By offering continuous support, this service helps individuals remain in their homes and avoid the risk of eviction or homelessness.
